Coaching Agreement 

We Agree to the Following Fees

 

Women’s Behavioral Health Coaching - $120 per one hour session

Men’s Life Coaching - $90 per one hour session

Marriage Coaching & Mentoring  - $150 per 90 minute session

Payment is due at the beginning of each session.

Coaching is an ongoing relationship between a coach and a person who desires coaching. We both agree that:


  1. Coaching is not therapy, counseling, advice giving, mental healthcare, or treatment for substance abuse or any addictive behavior. The coach is not functioning as a licensed mental health professional, therapist or counselor, and coaching is not intended as a replacement for counseling, psychiatric interventions, treatment for mental illness, recovery from the past abuse, professional medical advice, financial assistance, legal counsel, or other professional services.

  2. Coaching is for people who are basically well adjusted, emotionally healthy, functioning fairly effectively, and wanting to make improvements in there live.

  3. Coaching is designed to address issues the person being coached would like to consider. These include (but are not limited to) personal foundation, career development, relationship enhancement, spiritual growth, healthy lifestyle management, life balance, decision-making, and achieving short-term or long-term goals.

  4. Coaching will be an ongoing relationship that may take a number of months, although either party can terminate the relationship at any time. Some or all of the coaching may be through telephone contact.

  5. Coaching can involve brainstorming, values clarification, the completion of written assignments, education, goal-setting, identifying plans of action, accountability, making request, agreements to change behavior, examining lifestyle, and questioning.

  6. Coaching is most effective when both parties are honest and straightforward in their communication.


    Coaching is a confidential relationship and the coach agrees to keep all information strictly confidential, except in those situations where such confidentiality would violate the law.


  7. Coaching assumes that each person in the relationship is guided by his or her values and beliefs. Your coach is a follower of Jesus Christ and is in line with all the teachings of the Catholic Church. He/she seeks to live in accordance with this commitment. Your coach is honest in making this revelation and respects the different values and beliefs of others. While your coach does not seek to impose or pass judgement on another, each potential client should be aware that his/her coach will follow faithfully the teachings and ethics of said Catholic Faith and will never coach or assist anyone in pursuing goals that go against Catholic teachings.


We Agree to following session fees:

Women's Behavioral Health Coaching - $120 per one hour session

Men's Life Coaching & Mentoring - $90 peer one hour session

Marriage Coaching & Mentoring - $150 per 90 minute session

Payment is due at the beginning of each session.


  1. If I do not cancel any session with 24 hours notice I will forfeit the session fees.

  2. I (the person being coached) am committed to creating an alliance with my coach,

  3. Each of the people whose signatures appears below agrees that this agreement represents our mutual understanding of the coaching relationship.


    WOMEN'S BEHAVIORAL HEALTH COACHING


Behavioral Health Coaching (BHC) at first glance can seen similar to psychological counseling, however these are distinct services. BHC at NK Catholic Coaching is future focused, primarily attempting to identify and eliminate any relational gaps that may have been caused by past wounds a person has experienced that hinder deepening relationship with Christ who is the source of all healing. While evidence based traditionally therapeutic techniques will be taught and utilized, the goal is to coach clients through the process of learning and implementation. The expectation is that the client is emotionally healthy enough to implement these skills without becoming stuck. While Nellie does remain credentialed and certified as a therapist, her role here will strictly be as a Certified Catholic Behavioral Health Coach utilizing an integrative approach that is influenced by her counseling, spiritual direction, and marriage mentoring certification and training.


By Initialing each item listed below you attest that you have read, agree, AND understand the following:



  1. I understand that should it be determined that I am or become clinically depressed, I will be asked to pause coaching until an appointment is set up with a clinical therapist/counselor.

  1. I understand that should I become suicidal or experience suicidal ideation, I will be asked to pause coaching until an appointment is set with a clinical therapist/counselor.

  1. I understand that should I begin to experience a traumatic event that is determined needs processing, I will be asked to pause coaching until an appointment is set up with a clinical therapist/counselor.

  1. I understand that should I begin to experience clinical level anxiety, including but not limited to, severe (level of severity to be determined by Nellie Krawczynski) OCD/Scrupulosity, excoriation disorder, Trichotillomania, Post Traumatic Stress Disorder, I will be asked to pause coaching until an appointment is set up with a clinical therapist/counselor.

  1. I understand that should any already processed past/prior trauma or traumatic event be triggered in the present time-that needs reprocessing, I will be asked to pause coaching until an appointment is set with a clinical therapist/counselor.

By signing I understand that I have read and agree to the above stipulations and will disclose any mental health issues mentioned above should they arise.


DISCLAIMER: The client is the sole decision-maker in the coaching process. Any and all actions or consequences resulting from the coaching sessions are the responsibility of the client. The client releases the coach of all liability pertaining to the services rendered in the coaching relationship.

  1. How easy is it for your to recognize and talk about your feelings? For instance, do you clearly know when you are happy, sad, hopeful, grieving, angry, longing, out of control, ect ? would you be willing share such feelings openly in coaching/ marriage mentoring?
